The game is really short if you already know where to go. The good thing is that if you sprint through the entire game it will only take you an hour. You can only carry two batteries for your camera, the spawn rate of spare batteries is reduced, boss enemies kill you in one hit and normal enemies in two hits. So if you die, you will have to start from the very beginning and all your progress (collectibles etc.) will be lost.

Insane mode is based on Nightmare difficulty, but there are no checkpoints and you cannot save the game. The reason is that if you die on insane difficulty, all of your collectibles will be lost and you will have to start from scratch. It's best to create new save games regularly, so if you miss a collectible you can always go back.
